1. Compile all information regarding your medical treatment. Upon applying for Social Security disability, you will be asked what medical treatment you have been receiving and who are your medical providers. It is very important to know not only what doctors you are seeing including the doctor’s full name, address, and phone number but also to know when it is that you started seeing the doctor and any important dates associated with your treatment.
2. Be Thinking About Your Work History: Social Security will want to know what work you have done for the last 15 years. You will be provided with a work history form that asks for a detailed account of the work that you have done for the last 15 years including how much weight you had to lift, how long you had to stand, walk, or sit, and any supervisory duties that you had. It is important that you take your time when completing this section of your application as one of the first things Social Security will do when assessing your ability to work is to look to see if you could return to any job you have done within the last 15 years.
3. Ask your Doctor if he or she would be willing to support you filing for disability. Social Security may ask you if your doctor knows that you are filing for disability. They can also provide you with forms for your doctor to complete. It is important to have a discussion with your doctor regarding you filing for disability and whether he or she would be willing to provide you a statement supporting your disability claim.
